I was a Specialist Inspector in Human Factors at the British HSE (Health & Safety Executive) for many years. After creating a “Top 10” of human factors topics in 2001, our specialist team structured guidance, inspections, investigations and enforcement around these ten topics. This structure has since been adopted by many industries, professional associations, consultants, regulators and academics worldwide.
